Thank you for your participation in our Religion & series. In order to receive your $250 honorarium, please fill out the Payee Certification Form and return it to Nate Wynne (nwynne@iu.edu). You do not need to fill out the Check Request Form Number.

Also, you will be receiving a separate email from the IU Purchasing department with a link to get you set up as an IU Supplier. This separate email will come within a few business days of completing the form. You will need to create an account as an IU Supplier so that your honorarium for payment can be submitted in the system.
